Instructions
- Please download and fill out a GRAMA request form. It is a fillable PDF file. You will need Adobe Reader to view and fill out the PDF form.
- The form has a submit by Email button where the form data will be submitted by email. You can also print and then fax or mail the completed form to the number or address listed on the bottom of the form. If you have any problems with submiting the form please email our web contact.
